ABSTRACT

In recent years, organ donation and transplantation have entered a stage of steady development in China. Nevertheless, the shortage of transplant organs and the contradiction between supply and demand of organs are still the bottlenecks to achieve the strategy of "self-sufficiency in organ transplantation" advocated by the World Health Organization (WHO). The key reasons for donor loss described in the "critical pathway of organ donation" defined by the WHO include the identification and referral of potential donors and the maintenance and repair of organs. Smooth development, high efficiency and high-quality development of organ donation cannot be achieved without the support of intensive care medicine, which are highly associated with the cognition, recognition and participation of intensive care unit(ICU) staff. In this article, research progress in ICU staff’s cognition, attitude and willingness for organ donation were reviewed and relevant influencing factors were discussed, aiming to offer targeted suggestions on how to resolve these difficulties.

ABSTRACT

Objective At present, the novel coronavirus pneumonia (COVID-19) pandemic is still raging in certain regions around the globe, and the prevention and control of the pandemic should be strengthened. Under the challenges of respective social environment and allocation of medical resources, and support from the inertia and inherent productivity of the system on which the industry depends, extensive attempts are being delivered to push forward the work of organ donation and transplantation in each country. Under the guidance of national experts and committee members, Shanxi Provincial Human Organ Procurement and Allocation Service Center was established on August 28, 2018 approved by the former Shanxi Provincial Health and Family Planning Commission. It is the only independent non-profit medical institution in Shanxi Province. In this article, the system construction of citizen's organ donation and transplantation fitting national and provincial conditions was further explored according to the data analysis of organ donation and transplantation in the United States and Spain during the COVID-19 pandemic combined with the implementation of organ donation work in Shanxi Provincial Human Organ Procurement and Allocation Service Center.

ABSTRACT

Hepatocellular carcinoma (HCC) has the features of high incidence rate, low survival rate, poor treatment outcome, and complex pathogenesis. In recent years, many studies have shown that long non-coding RNA (lncRNA) MALAT1 is upregulated in HCC and can promote the proliferation, invasion, and metastasis of HCC cells, and it can also guide the diagnosis, prognostic evaluation, and treatment of HCC in clinical practice. This article reviews the current status of research on lncRNA MALAT1 in HCC and discusses its expression pattern, mechanism of action, and clinical significance in predicting and monitoring the progression of HCC, so as to gain a deep understanding of the role of lncRNA MALAT1 in the progression of HCC. It is pointed out that lncRNA MALAT1 is expected to become a potential biomarker for the diagnosis and prognostic evaluation of HCC and may be used as a therapeutic target in clinical practice.

Objective@#To analyze the role of thyroid imaging reporting and data system(TI-RADS), contrast-enhanced ultrasound(CEUS), fine needle aspiration cytology (FNAC) and tumor proliferation related genes in the early diagnosis of thyroid micro-papillary carcinoma(PTMC) and risk assessment of early metastasis.@*Methods@#From May 2018 to May 2019, a total of 140 patients with Thyroid micronodules for surgical resection and pathological diagnosis of benign or malignant into the Seventh People′s Hospital Affiliated to Shanghai University of Traditional Chinese Medicine for the retrospective study.There were 90 cases in benign group and 50 cases in malignant group.The levels of TI-RADS, CEUS enhancement mode, peak intensity (PI) and cyclin D1 (CCND1), cell nuclear Proliferating Antigen (PCNA) and vascular endothelial growth factor (VEGF) were compared between malignant and benign groups, VEGF) mRNA expression level.The positive rate of FNAC, TNM stage, capsule invasion and lymph node metastasis were evaluated.@*Results@#The percentage of class four and more by TI-RADS grade in malignant group was significantly more than benign group((92.0% (46/50) vs.5.6% (5/90), χ2=103.718, P<0.001), more early low enhancement by CEUS(86.0%(43/50) VS.6.7%(6/90), χ2=91.328, P<0.001) and PI value higher than benign group, too((6.79±1.88) VS.(5.32±1.46), t=4.968, P=0.008). The positive rate of FNAC in malignant group was 92.0% (46/50). FNAC was positive in 42 cases of TNM stage, 9 cases of capsule invasion and 6 cases of lymph node metastasis.The pathological positive rate after resection was 50 cases of TNM stage, 10 cases of capsule invasion and 6 cases of lymph node metastasis.The expression of CCND1, PCNA and VEGF mRNA in malignant group was significantly higher than that in benign group((0.5624±0.134) VS.(0.213±0.097), t=15.639, P<0.001; (0.453±0.126) VS.(0.186±0.056), t=20.253, P<0.001; (0.633±0.159) VS.(0.252±0.097), t=31.265, P<0.001).@*Conclusion@#Ultrasound-guided FNAC is used to determine TNM staging, capsule invasion and lymph node metastasis, CCND1, PCNA and VEGF expression level, contrast-enhanced mode and peak intensity value measured by quantitative evaluation software have good accuracy for early diagnosis of PTMC, and are consistent with the pathological results of hand surgery.

MicroRNA-223 (miR-223) is located on chromosome X, and is highly conserved in the process of evolution. In recent years, many studies have shown that miR-223 is abnormally expressed in a variety of digestive system tumors, such as esophageal cancer, gastric cancer, liver cancer, pancreatic cancer and colorectal cancer. MiR-223 can participate in the proliferation, apoptosis and invasion of tumor cells through a variety of signal pathways, which is expected to become a marker for the diagnosis and prognosis of digestive system tumors.

ABSTRACT

Objective To investigate the relationship between primary platelet parameters and early acute rejection (AR) after renal transplantation.Methods From October 2015 to June 2017,112 patients with renal failure underwent allograft renal transplantation in the Second People's Hospital of Shanxi Province were selected as the research objects,according to the preoperative and postoperative 30d in platelet parameters based on the occurrence of AR they were classified into the observation group (23 cases),no rejection for the control group (89 cases),analysis the difference of two groups of patients with platelet parameters [peripheral blood platelet count (PLT),platelet hematocrit (PCT),mean platelet volume (MPV) and platelet volume distribution width (PDW),the ratio of large platelet (P-LCR)] at preoperative (T0),postoperative 5 d (T1),10 d (T2) after operation,postoperative 15 d (T3) 15,30 d (T4).The correlation between PLT,PCT,MPV,PDW,P-LCR and acute rejection after renal transplantation was also analyzed.Results Compared with those of preoperative,the PLT [(122.06 ± 49.67) × 109/L vs.(185.26 ± 63.49) × 109/L)],PCT[(0.46 ± 0.28) % vs.(0.34 ± 0.06) %)] in T1,T2 of the two group after surgery declined (P < 0.05).MPV,PDW and P-LCR were increased in both groups after surgery (P <0.05),and the observation group were higher than that in the control group MPV[(13.69 ± 0.59) vs.(12.24 ±0.42),(16.08 ±0.61) vs.(12.52 ±0.57)],PDW[(13.26 ± 1.19) vs.(12.13 ±2.14),(15.46±3.15) vs.(12.72 ±2.09)],P-LCR[(30.57 ±7.29) vs.(28.13 ±6.34),(33.01 ±3.98) vs.(29.13 ± 6.91)] at the baseline of T1 and T2 (all P < 0.05).Correlation analysis showed that the level of MPV,PDW,P-LCR and the occurrence of acute rejection after kidney transplantation were positively correlated (r =0.464,0.606,0.512,all P <0.05),while the PLT,PCT has nothing to do with the occurrence of acute rejection after kidney transplantation.Conclusion MPV,PDW and P-LCR can indirectly reflect the occurrence of AR after renal transplantation,and the trend of monitoring the three indexes of MPV,PDW and P-LCR after operation is an auxiliary diagnostic method for postoperative AR.
